The Ambulatory Care Pharmacist is responsible for provision of comprehensive outpatient pharmacy services in LMH Health ambulatory care clinics. The pharmacist works collaboratively with prescribers under collaborative practice agreement to facilitate optimal patient outcomes regarding safe medication use. Disease states may include, but are not limited to: anticoagulation, asthma, COPD, diabetes, dyslipidemia, heart failure, hypertension, osteoporosis, and tobacco cessation.
