Pharmacy patient Care Process

Established by the Joint Commission of Pharmacy Practitioners (JCPP), the Pharmacists’ Patient Care Process (PPCP) serves as guidance for consistent delivery of patient care for pharmacists in all practice settings. It was developed by a group of national pharmacy organizations that work under the direction of JCPP. Its goal is high quality, cost-effective and accessible health care for patients. The JCPP feels this goal is best achieved through team-based, patient centered care. PPCP describes how pharmacists collect, assess, plan, implement, and follow-up through monitoring and evaluation of patients as part of a cohesive health care team.
