The Ambulatory Care Pharmacist is responsible for provision of comprehensive outpatient pharmacy services in LMH Health ambulatory care clinics. The pharmacist works collaboratively with prescribers under collaborative practice agreement to facilitate optimal patient outcomes regarding safe medication use. Disease states may include, but are not limited to: anticoagulation, asthma, COPD, diabetes, dyslipidemia, heart failure, hypertension, osteoporosis, and tobacco cessation. This consists of ensuring drug therapy that results in safe, effective, and cost conscious use of pharmaceuticals. Must demonstrate excellent interdisciplinary communication and exhibit strong educational and precepting abilities.
